The red skull needs to face up from the perspective of the player. This weapon should get a red energy flare around the skull when firing, somewhat like the Mage's master weapon in Hexen, and the eye sockets should light up also. The player's perspective on the weapon should start above the two skulls you see on the lower portion, so all he sees is the main staff, the larger top portion with the claw settings, and the red skull on top.
